How do I write a trading plan?

I have stuff in my head. Like the IB, and monitoring it. I use a 150 tick chart for entry's, and a 1000 BLB chart for my large time frame. I watch major support and resistance, and swing highs and swing lows. I use HH, LH and vise versa. I just do not know how to put this all together. What should I define first? Hope this makes sense to someone who went through this and got to the other side. I know I'm not explaining myself very well, and I guess that is part of my problem. It's in my head, but I notice that I do forget things, as silly as that sounds, I do forget really imporatnt things sometimes. Anyways anyone's help I appreciate.

Traders are very visual people. Post your Charts with all your bells and whistle indicators on it and you'll be surprise to all the opinions and advise you get on how to come up with a trading plan.
